Countryside also bills Medicare part B for therapies and certain
medical supplies. Supplies covered by Medicare are extremely limited
but include influenza vaccines; Pneumonia vaccines; Physical, Occupational,
and Speech Therapy, and Specialized Bandages.
Admission requirements: Completed
FL-2 and PASARR and a personal interview for evaluation to determine
if we can adequately care for the proposed resident.
Subsequent contacts: We ask families
to keep us posted monthly if they are interested in remaining on
the waiting list. Also, to notify us of any changes in condition
or living condition or living arrangements.
