Countryside also bills Medicare part B for therapies and certain medical supplies. Supplies covered
by Medicare are extremely limited but include Influenza vaccines; Pneumonia vaccines; Specialized
Bandages; Physical, Occupational, and Speech Therapy.

Admission requirements: Completed FL-2, PASARR and a personal interview for evaluation
to determine if we can adequately care for the proposed resident.

Subsequent contacts: We ask families to keep us posted monthly if they are interested in
remaining on the waiting list. Also, to notify us of any changes in physical condition or living
arrangements.
